Click on  "Register"

Enter your information in the first 4 categories (Personal Information, Billing Address, Shipping Address and Contact Information).

Under "Username and Password" follow these instructions:

     Account Type: Patients

     Username: select your own

     Account Number: obtain this information from CornerStone Chiropractic

     Password: select your own

     Confirm Password:  re-type your password

Click on "Submit"

You are now logged on and are able to purchase your supplements.
